And I understand that in the event that the above facts are found to be falsified, I subject myself to immediate termination of my benefits and I will be disqualified from applying to any programs by Healing Angles Organization for a period of at least two (2) years. 

  By checking, I understand that submiting my application does not mean I am automatically qualified to participate in the next Gardening class, and that my application is subject to evaluation. And I agree to submit any additional documentation that Healing Angles Organization may require .
